The Form W-9 is used to provide your correct Taxpayer Identification Number (TIN) to the person/entity who is required to file an information return with the IRS. 

REQUEST CERTIFICATES OF INSURANCE

Upon acceptance of our Janitorial Proposals, your business/entity shall be listed as addition insured to our Workers' Compensation, General Liability/Umbrella and Auto (Owned and Hired) Insurance Policies.
